Checklist Antes de Enviar

- Obtener fecha exacta de Certificate of Formation (campo #2)
- Confirmar nombre legal exacto (campo #1)
- Contactar Franchise Tax Section: (302) 739-3073 — confirmar back taxes adeudados
- Pagar back taxes + penalties + interest (si aplica)
- Preparar check por $200.00 + taxes adeudados a "Delaware Secretary of State"
- Incluir cover letter con nombre, direccion y telefono
- Firmar el formulario oficial (no este borrador)
- Enviar por correo a: Division of Corporations, 401 Federal St, Suite 4, Dover, DE 19901
NOTA IMPORTANTE: Si Delaware ya cancelo la entidad automaticamente por impago (3+ anos), puede que NO sea necesario presentar este formulario. El ICIS status confirmara si la entidad ya esta "VOID" o "CANCELLED". En ese caso, la deuda de Franchise Tax puede seguir pendiente pero el filing de cancelacion no aplica.
